Build a skeleton RADIUS request using information from the config file.

Parameters
- rh
a handle to parsed configuration.
- data
a pointer to a SEND_DATA structure.
- code
one of standard RADIUS codes (e.g., PW_ACCESS_REQUEST).
- server
the name of the server.
- port
the server's port number.
- secret
the secret used by the server.
- timeout
the timeout in seconds of a message.
- retries
the number of retries.
